Protecting Your Privacy

Handled complies with the relevant laws based on the Privacy Act 1988 to protect the privacy of your information. We have strict policies on how we collect, use, store, disclose and dispose of your personal information.

Your information can only be viewed by authorised staff who have signed a confidentiality agreement as part of their employment contract.

This privacy policy aims to clearly outline:

  • What personal information we collect
  • Why your personal information is being collected and how it will be used by Handled.
  • Who we disclose your personal information to
  • How your information is stored and disposed of
  • The circumstances in which we share it with third parties
  • How our patients can correct and access their own personal information
  • How our patients can address any privacy concerns

What is "Personal Information"?

Personal information is any information that can be used to personally identify you. This may include you name, address, email address, telephone number, or occupation.

Sensitive information is personal information that includes information or an opinion about an individual, such as health and genetic information.

Handled treats your personal and sensitive information in the same way, so for the remainder of this document, both will be referred to as “Personal information.”

What Personal Information do we collect?

The Personal Information that Handled collects and stores may include:

  • Your name, address, date of birth and contact details
  • Your next of kin/ carer/ support coordinator’s name and contact details
  • Information about your health condition, functional status, medical history, social and family history, risk factors, lifestyle factors, medications, allergies, adverse events and treatment you may have already received.
  • Your ethnic background, occupation/ job title
  • Third party details such as NDIS number, TAC, Healthcare Card number, Workcover Numbers, Medicare number or DVA number for identification and claiming purposes.
  • The name of any health service provider or medical specialist to whom you are referred, copies of any letters or referrals and copies of any reports
  • Private health fund details
  • Any additional information about you that you may provide to us directly

What is your Personal Information used for?

Handled only collects your Personal Information that is reasonably necessary for us to provide health services to you.  It may be also be used:

  • To provide you with the best possible health services
  • To make decisions about your care in partnership with you
  • To process and respond to any complaint made by you
  • For financial claims and payments
  • For heatlh surveys and quality assurance
  • For accreditation
  • For practice audits

For staff education and training

How we collect your personal information:

Handled will collect your personal information in a fair, lawful and non-intrusive way including:

  • you complering our patient referall forms online or on hard copy
  • In person during a consultation at our centre or in your home
  • Over the phone
  • Via Telehealth video consultation
  • By email
  • By SMS
  • By social media

 

We may also collect personal information from third parties including:

  • Information provided on your behalf by your care giver, power of attorney, guardian, parent, next of kin, support person or support coordinator with your consent
  • From health service providers to whom you are referred
  • From health service providers who refers you to our service
  • Other involved healthcare providers such as specialists, allied health professionals,  hospitals, community health services, pathology and diagnostic imaging services
  • From your employer or prospective employer
  • From third parties such as NDIS, workcover, medicare, your private health fund or law enforcement agencies

Who might we disclose your personal information to?

  • Your referring doctor, health professional or support coordinator
  • Other health care professionals such as GP or radiologist to confirm your diagnosis
  • Other health professionals that we may refer you to with your consent, such as physiotherapist, psychologist or occupational therapist.
  • Your employer or respective employer in the case of work related treatment
  • Professional advisors such as accountants, solicitors, business advisors and consultants
  • IT system administrators, payment processors, debt collectors, business and marketing third parties
  • Statutory requirement to lawfully share certain personal information, such as mandatory notification of certain diseases
  • Court subpoenas required or authorised by law
  • When necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent

Your personal information will not be shared, sold, rented or disclosed other than described in this policy or as permitted under the Privacy Act 1988.

What happens if you do choose not to disclose your information?

It is your choice as to whether you wish to disclose your personal information, however, if you choose not to, we may not be able to provide you with the best possible services. You may also have an inaccurate diagnosis, assessment of function or incomplete treatment.

At any time,you can decide not to have any personal information disclosed to a third party as stated or allow any third party access to or be informed about your personal information or circumstances.

If you decide to be treated with an alternative health care provider who requires access your personal/health information, you will need to provide us with written authorisation to transfer this information to the alternative health service.

How we store and protect your personal information:

We store your information in an electronic record system that is password protected and has restricted access to authorised staff only.

In some cases your personal information may be stored at Handled as hard copy records, paper records, as x-rays, CT scans, photos or videos. We store all of this data securely and have strict protocols to ensure it is protected from misuse, loss, interference or unauthorised access.

When your personal information is no longer necessary for healthcare purposes, we will responsibly de-identify and securely dispose of it.

How you can access and correct your personal information:

If you believe that your personal information is inaccurate, you may contact us at any time via phone or email to request it to be corrected or updated.

Under the Privacy Act you have a right to access your personal information. We require your request to be completed in writing and may require identification from you before releasing your information.

You may be charged a small administration fee to cover the cost of providing information to you, and will be advised of this on your request for the information.

In some cases we may not be able to provide your with the requested information if it breaches someone else’s privacy or results in a breach of confidentiality.

You have a right to request to stop receiving unwanted direct market material.

How you can lodge a privacy related complaint and how it will be managed at our practice:

If you feel that your privacy has been breached, please get in touch with us via email so we can investigate your complaint. We will endeavour to notify you in writing as to the outcome of our investigation within 30 days of the original complaint.

If you remain dissatified with our response, you have the option to reach out to us directly to address any additional concerns. Alternatively, you can lodge a complaint with the Australian Information Commissioner by visiting www.oaic.gov.au or calling 1300 363 992.

Privacy and our Website

Our website may contain links to other third party sites that are not operated by us. We do not guarantee website links or policy of authorised third parties. We advise you to review the Privacy Policy of every website you visit.

Given that our website is connected to the internet, which inherently lacks security, we cannot assure the safety of informatino your transmit to us online. Additionally, we cannot guarantee that the date you provide won’t be intercepted during internet transmission. Therefore, any personal or other information you share with us online is done so at your own risk.
