Referral Information
Please contact your private health insurer to determine if you are covered for “occupational therapy” extras which will allow you to claim a rebate following payment for therapy.
A Medicare rebate is not available unless you GP has deemed you eligible for a Chronic Disease Management Plan (CDM), also known as an enhanced primary care plan (EPC), and completed the necessary documents.
All compensable patients (workcover QLD, DVA, third party insurers) require a referral from a treating surgeon of GP.
To assist us with assessment and treatment of your condition, please bring all relevant test results or imaging e.g. x-rays, MRI results to your appointment.
