A Medicare rebate is not available unless you GP has deemed you eligible for a Chronic Disease Management Plan (CDM), also known as an enhanced primary care plan (EPC), and completed the necessary documents.
All compensable patients (workcover QLD, DVA, third party insurers) require a referral from a treating surgeon of GP.
